Nursing Home Medicaid Information

When a patient’s skilled nursing facility (SNF) stay exceeds Medicare’s coverage and private funds or a long-term care insurance policy is unavailable or exhausted, then the individual must apply for Medicaid funding. This could require transferring the patient to a Medicaid Certified SNF. 

When, instead, an individual is living in the community with deteriorating health that requires moving into a SNF, a physician must order the admittance (regardless of the source of payment).
