What is The Day Center of Evergreen Commons?
A Supportive Space for Independence and Connection

Did you know that Evergreen Commons offers a state-of-the-art adult day services program just down the street from our main campus? The Day Center of Evergreen Commons is designed for individuals who need assistance or are at risk of social isolation. Our specially trained staff members create a warm environment for participants and provide relief for care partners during the day—enabling individuals to live at home and remain independent in the community.
What We Offer
- Therapeutic Activities
This program’s goal is to delay or prevent institutionalization due to health and cognitive impairments. These therapeutic activities are designed to maintain or improve functioning and well-being. They range from creative arts to education, music, physical/gross motor skill building, socialization activities, and spiritual activities too. - Socialization
Our programming encourages self-esteem and relationship-building through shared activities and family-style dining, strengthening connections among participants. - Health Monitoring
Our nursing team, which includes a Registered Nurse and a Health Support Coordinator, conduct assessments, monitor vital signs, administer medications, and communicate with families and medical providers regarding health-related observations and concerns. - Respite Care
A primary reason families pursue adult day services is to relieve family care partners. Our program offers care partners peace of mind, knowing their loved one is safe so they can take time for self-care, manage work obligations, or run errands.
The Day Center of Evergreen Commons provides emotional support and resources for both participants and care partners. Flexible schedules are available, and funding support may be accessible through local agencies on aging—based on eligibility.
