Aging in Place with the Help of In-Home Care and Adult Day Services

Local Resources to Support Independent Living

Aging in place allows people to remain in the comfort of their own home as they age. It often provides a greater level of independence and freedom in a familiar and comfortable environment. While this lifestyle can improve quality of life, it requires careful planning and coordination. But with the right support services, both inside and outside the home, aging in place can be an effective way to maintain independence and well-being as you or your loved one age.

If choosing to age in place, it’s important to think of common safety concerns. Home modifications, such as grab bars or wheelchair ramps, can help reduce risks and keep the home environment as safe as possible. There are also ways to bring services to the home, like in-home personal care, home health care, housekeeping, and meal delivery for extra supervision and hands-on support.

The Evergreen Commons Resource Center is a hub for information on these services, helping you find the best options to stay at home as long as possible. For a detailed list of the resources available to help you or your loved one age in place, see our article Start Here: Accessibility.

Social Engagement and Adult Day Services

Social engagement and health monitoring programs outside the home, like the Day Center at Evergreen Commons, are designed for those who need social support or help managing daily life. Adult Day Services help reduce the risk of social isolation and delay the need for long-term care by offering a range of beneficial programs outside the home. These services help individuals stay independent while still being active in the community. Some of the benefits of using The Day Center at Evergreen Commons are:

  1. Therapeutic Activities: Activities designed to maintain or improve functioning, including arts, music, emotional regulation, and physical activities. These are tailored to address cognitive and health challenges and prevent institutionalization.
  2. Socialization: Programming encourages self-esteem and relationship-building through shared activities and family-style dining.
  3. Health Monitoring: A nursing team, including a Registered Nurse and Health Support Coordinator, conducts cognitive assessments, monitors vital signs, administers medications, provides dietary management and communicates all observations and concerns with families and medical providers.
  4. Respite Care: One of the most common reasons people look into adult day service programs is to provide relief for family care partners. Knowing that loved ones are safe and supported offers peace of mind, and gives care providers time for self-care, work obligations, errands or rest.
